Foster Care Admissions

Admission Criteria

Adult foster care provides residential care and support for individuals who need supportive housing and services due to physical, emotional, developmental or mental health reasons.

Arrowhead House Foster Care provides, but is not limited to the following:

  • Lodging
  • Individualized care plans
  • Supervision/ protection
  • Medication administration/ management
  • Household services
  • Groceries and meals
  • Transportation to medical appointments
  • Weekly/ monthly activities
  • 24/7 Staffing


To qualify for Adult Foster Care services through Arrowhead House Foster Care you must be:

  • Over the age of 18 and under the age of 65 (unless approved for an Elderly Waiver through CADI)
  • Eligible for Medical Assistance (MA) based on disability or other eligibility status
  • Assessed and have an open MnCHOICES assessment completed
  • Independent in ADL’s such as showering, toileting, etc.
